One primary use for the Grad. ND filter is to darken the sky when it would otherwise be overexposed.  The bottom image is a regular shot of the Vancouver skyline, while the top image has utilized a Grad. ND filter.  In the unfiltered image the sky is completely overexposed and washed out, whereas in the G.ND filtered image the sky has much richer colour saturation and detail.

The graduated neutral density filter can be used to reduce the amount of light in one area of the image.  The denser half of the filter will reduce light while the other half is transparent, with a gradual transition between the two.  The filter can be shifted and rotated in the filter stage to affect only the portion of the image that is desired.
